Child Health Assessment
This form is to be completed by your child's physician. It verifies that your child has had the state-required immunizations and has had a physical examination within the last calendar year. This must be returned by the first day of school. Playschool requires that children who have not been immunized in an age-appropriate manner prior to enrollment show evidence in writing of an appointment for immunizations of the parent must complete a waiver form.

Emergency Contact
Playschool requires an Emergency Contact Form for each child. Please complete this form as soon as you receive it, and return it by the first day of school. It informs the staff who to contact in case of emergency, if your child has allergies, and the name of your child's doctor. It must be reviewed, amended (if necessary) and re-signed every six months.

Field Trip Permission
Your signature is required on the Emergency Contact Form, giving permission for all Walks and Trips. You will be notified in advance of all trips.
